Duties/Responsibilities:

With FT staff build a Personal Health Portfolio with everything in place, and every concern fully addressed.

• Offer patients enhanced access, including 24/7 availability for urgent needs and timely appointments
• Conduct thorough, extended patient appointments
• Provide oversight and care coordination with specialists and other healthcare providers to ensure a seamless patient experience
• Serve as patients’ primary health advocate, delivering personalized planning, primary care, and health optimization
• Manage chronic conditions and empower patients to take an active role in their health through education
