Position Overview

We are seeking a full-time Family Medicine (FP) or Internal Medicine (IM) physician to join our practice and become a successor physician in our long-established clinic.

Job Requirements

• Board-certified in Internal Medicine (IM) or Family Practice (FP)
• Minimum of 5 years of experience
• Detail-oriented yet personable

Duties and Responsibilities

• Collaborate with full-time staff to build a comprehensive Personal Health Portfolio, ensuring every detail is in place and every concern is fully addressed.
• Offer patients enhanced access, including 24/7 availability for urgent needs and timely appointments.
• Conduct thorough, extended patient appointments.
• Provide oversight and care coordination with specialists and other healthcare providers to ensure a seamless patient experience.
• Serve as the patients’ primary health advocate, delivering personalized planning, primary care, and health optimization.
• Manage chronic conditions and empower patients to take an active role in their health through education.
